Long-term mortality and complications of Björk-Shiley spherical-disc valves--a life table analysis.

چکیده
Bj6rk-Shiley Spherical-Disc Valves have been one of the most commonly used mechanical valves in the past 20 years. The long-term mortality and complications of these valves is therefore of great interest to the life insurance medical director. This paper presents an unusually complete set of data and additional follow-up of two earlier separate articles on long-term follow-up of aortic and mitral valve replacements which were reported in the 1990 Medical Risks (see Abstracts No. 669 and No. 672). From the mortality abstract perspective the article is excellent in allowing comparison of results by the life table method with the results from the survival curves which are on good-sized graphs.

Age thresholds for prophylactic replacement of Björk-Shiley convexo-concave heart valves. A clinical and economic evaluation.
BACKGROUND Björk-Shiley convexo-concave heart valves have an increased risk of mechanical failure. One might consider prophylactic rereplacement as a preventive measure to avert the disastrous consequences of these failures. We investigated the effect that prophylactic rereplacement has on survival of individual patients and on the medical costs. METHODS AND RESULTS Quantitative estimates for...

Twenty-four children (ages 1 to 18 years, mean 12.2 years) underwent 27 operations for aortic, mitral, or combined aortic and mitral valve replacement. There was 1 operative death. Of the 23 operative survivors (12 aortic, 8 mitral, 3 combined valve replacement), only 5 were given warfarin for long-term anticoagulation. The remaining 18 (10 aortic, 8 mitral valve replacement) were given aspirin...
متن کاملSummary and recommendations.
It has been estimated that up to 7,000 patients with Björk-Shiley Delrin (BSD) heart valves may be alive as of January 1996, the range of implant durations being from 15 to 27 years. The clinical question was whether these BSD valves could be expected to continue to function satisfactorily or if prophylactic replacement might be warranted. The data presented in this supplement suggest that clin...
متن کاملAortic valve replacement in patients over 70 years.
Twenty-seven patients 70 years of age or more were operated upon with aortic valvular replacement with an early mortality rate of 7%. No more patients died during a mean follow-up period of 28 months. The Björk-Shiley standard tilting disc valve was implanted in 15 cases and the new convexo-concave model in 12 cases. Sixty-seven per cent of the patients had narrow aortic roots (21 and 23 mm pro...
متن کاملThrombotic obstruction of the Björk-Shiley valve: the Glasgow experience.
Thrombotic obstruction is the most feared complication of the Björk-Shiley tilting disc prosthesis. From 1971 to 1982 1186 Björk-Shiley valves were implanted in 900 patients. There were 93 deaths in hospital. Eight hundred and seven patients have been followed for a total of 4146 patient years; 14 patients were lost to follow up. Nineteen cases of thrombotic obstruction were identified at necro...
